Comprehensive Housing Support Tailored to Your Needs

At Gateway Housing, we are dedicated to empowering individuals with disabilities, seniors, and those facing housing challenges to achieve independence and stability. Our services are customized to meet the unique needs of every client, helping them lead dignified, self-sufficient lives.

Housing Stabilization Services

Our housing stabilization services are designed to assist individuals in finding, transitioning to, and maintaining stable housing. By partnering with local resources, we provide critical support for individuals with disabilities and seniors.

These services fall under the state plan Home and Community-Based Services (HCBS) and include:

  • Housing Transition Services: Guidance and assistance during the moving process.
  • Sustaining Services: Ongoing support to ensure housing stability.
  • Housing Consultation: Tailored strategies and advice to meet your housing needs.

Eligibility Criteria

To qualify for our services, Client must meet one of the Following Criteria:

  • Eligible for Medical Assistance (MA)
  • 18 years or older
  • Have a documented disability or disabling condition, including:
    • Aged, blind, or disabled (as defined by Title II of the Social Security Act)
    • Developmental disability or related condition
    • Mental illness, substance use disorder, or physical injury requiring residential-level care transitioning to the community
    • Learning disability as determined by the Department of Human Services (DHS)
  • Must require assistance in at least one of these areas due to a disability or long-term condition:
    • Communication
    • Mobility
    • Decision-making
    • Managing challenging behaviors
  • Housing instability evidenced by one of the following:
    • Homeless or at risk of homelessness
    • Transitioning or recently transitioned from an institution or licensed setting
    • At risk of institutionalization

We also provide services to individuals eligible for the following waivers:

  • Brain Injury (BI)
  • Community Access for Disability Inclusion (CADI)
  • Community Alternative Care (CAC)
  • Developmental Disability (DD)
  • Elderly Waiver (EW)
